Legal disclosures and patient rights.
Elora Behavioral Health · Effective January 2027
At Elora Behavioral Health, we are committed to providing compassionate, accessible, and high-quality psychiatric care in a safe, respectful, and supportive environment. We believe every patient deserves to be treated with dignity, honesty, and respect.
The following information outlines your rights as a patient, your responsibilities, and important disclosures regarding the services we provide.
1. Provider information and scope of services
Elora Behavioral Health is a psychiatric practice led by Suzanne Perakathu, APRN, PMHNP-BC, a board-certified Psychiatric-Mental Health Nurse Practitioner licensed as an Advanced Practice Registered Nurse in Texas.
Our services include psychiatric evaluations, diagnosis and treatment of mental health conditions, medication management, and ongoing psychiatric follow-up care.
Psychiatric nurse practitioners are not physicians or psychiatrists. They are advanced practice registered nurses with specialized graduate education and clinical training in psychiatric and mental health care. Prescribing and other clinical services are provided within applicable Texas laws and professional standards.
While psychotherapy is an important component of mental health treatment, our primary focus is psychiatric evaluation and medication management. Patients may be referred to qualified therapists, counselors, psychologists, or other healthcare professionals when clinically appropriate.
2. Patient rights
As a patient of Elora Behavioral Health, you have the right to:
- Receive respectful, compassionate, and nondiscriminatory care.

- Participate actively in decisions regarding your treatment.
- Accept or refuse recommended treatment, subject to applicable laws and clinical considerations.
- Receive care in an environment that respects your privacy, dignity, and personal beliefs.
- Have your personal and medical information protected in accordance with applicable privacy laws.
- Request access to your medical records and obtain copies as permitted by law.

- Express concerns or file complaints without retaliation.
- Discontinue treatment or request a transfer of care, subject to applicable clinical and legal considerations.
3. Patient responsibilities
We believe effective psychiatric care is built on communication, mutual respect, and collaboration.
Patients are expected to:
- Provide accurate and complete information regarding medical history, psychiatric symptoms, medications, and substance use.
- Inform the provider of significant changes in symptoms, health conditions, or medications.
- Participate in treatment planning and follow agreed-upon treatment recommendations.
- Take medications only as prescribed and communicate concerns or side effects promptly.
- Attend scheduled appointments and follow appointment cancellation policies.
- Maintain respectful communication with providers and staff.
- Fulfill financial obligations according to practice policies.
- Follow applicable medication management and controlled substance policies.
Failure to follow essential treatment or safety requirements may result in reassessment of the treatment plan or termination of the provider-patient relationship, consistent with applicable laws and professional obligations.
4. Privacy and confidentiality
Elora Behavioral Health is committed to protecting the confidentiality of patient health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA), applicable Texas laws, and other relevant privacy requirements.
Patient information may be used or disclosed for treatment, payment, healthcare operations, and other purposes permitted or required by law.
Patients have certain rights concerning their protected health information, including the right to request access, amendments, confidential communications, and certain restrictions on disclosures.
For complete information, please review our HIPAA Notice of Privacy Practices and Website Privacy Policy.
5. Informed consent and medication management
Psychiatric treatment is individualized based on each patient's clinical needs.
Medication recommendations are made after considering symptoms, medical history, potential benefits, risks, treatment alternatives, and relevant clinical information.
Patients are encouraged to ask questions and participate in treatment decisions.
Prescriptions, including controlled substances, are issued only when clinically appropriate and consistent with applicable federal and Texas laws.
A psychiatric evaluation or appointment does not guarantee that medication, including controlled substances, will be prescribed.
Additional monitoring, laboratory testing, medical records, referrals, or follow-up appointments may be required to support safe and effective treatment.
6. Telehealth services
Elora Behavioral Health may offer psychiatric services through secure telehealth technology when clinically appropriate and legally permitted.
Telehealth appointments are subject to applicable professional standards, patient location requirements, privacy safeguards, and prescribing regulations.
Patients should understand that telehealth has limitations, including potential technical disruptions and situations in which an in-person evaluation may be necessary.
The provider may recommend or require in-person care, referral, or a higher level of care when clinically indicated.
Telehealth services are not intended to replace emergency psychiatric or medical care.
7. Emergency and crisis services
Elora Behavioral Health is not an emergency psychiatric service or crisis response facility.
Website messages, appointment requests, emails, and routine patient communications should not be used for emergencies.
If you are experiencing a medical or psychiatric emergency, experiencing immediate danger, or are concerned about your immediate safety:
- Call 911 or go to the nearest emergency department.
- Call or text 988 to reach the Suicide & Crisis Lifeline for crisis support.
- Do not wait for a response from Elora Behavioral Health during an emergency.
8. Financial responsibilities and insurance
Patients are responsible for understanding their insurance benefits, including copayments, deductibles, coinsurance, and coverage limitations.
Insurance coverage and reimbursement are determined by the patient's insurance plan and applicable payer requirements.
Payment obligations, appointment cancellation fees, and other applicable charges are described in our Financial Policy.
Good Faith Estimate notice
Under the No Surprises Act, patients who are uninsured or choose not to use their health insurance have the right to receive a Good Faith Estimate of expected healthcare charges.
Elora Behavioral Health provides Good Faith Estimates upon request and when scheduling eligible services, in accordance with federal requirements.
